Abbey Wood Health Visiting Team, Greenwich Community Health Services
The team were nominated for "increasing their partnership working with children's centres." This has increased support available to children and their families and has allowed both services to plan to meet the needs of individual families. New activities developed include breastfeeding support groups and support sessions for antenatal mothers.

ACAN Team, Greenwich CAMHS
The ACAN team received a couple of nominations. One said that the team work together well, help each other for the good of their clients and are flexible and responsive to their need.

Alfred was nominated by a service user for being caring, and making the patients laugh.

Andy Smith, Trust Lead for Fitness and Sports
Andy is based at the Bracton Centre but in the last year he has been organising weekly sports sessions for service users at the Tall Trees day service for people with learning disabilities and challenging needs. Andy had a conversation with the team leader in the service who said there was very little in the way of specialist sports services for their clients and agreed to come over and start providing the sessions. Tall Trees entered a team into the trust football competition earlier this year at Charlton Athletic due to Andy.

Annie was nominated by a service user for being 'wonderful' and a 'great person'.

Anthony Hawkins, Danson Clinic at the Bracton Centre
A member of Anthony's team nominated him for being an excellent team leader and for taking an interest in every member of his team, ensuring they have job satisfaction and are able to provide excellent services to their patients.

Audiology and Newborn Hearing Screening Programme (NHSP) team, Greenwich Community Health Services
The Audiology and NHSP team were nominated by a team member for always going that extra mile for children and their families and for providing a service that they can all be proud of. 

Ayse Abdurrahman, The Tarn, Oxleas House
A colleague nominated Ayse for always being mindful of patients' needs. The nominator said: "nothing is too much trouble for her where her work colleagues are concerned." Another nominator said she deserved an award for: "Always putting those in her care at the forefront of her work."

Barbara Winters, Assessment and Shared Care Team
A service user nominated Barbara for always being kind, caring and compassionate on reception.

Barefoot Lodge Team, Greenwich Complex Needs Recovery Team
Barefoot Lodge is an inpatient community rehabilitation unit for 15 people. The team were nominated for the work they have done to develop their model of care. 80% of patients have been discharged in the past 12 months. Developments include a family inclusive practice clinic.

Betts Ward Team (triage), Green Parks House
The team's multidisciplinary approach to care planning and timely and efficient triaging of patients has resulted in efficiency savings and greater satisfaction of patients. 

Bexley Child Health Department, Bexley Community Health Services
A member of the team wanted the Bexley Child Health Department recognised for learning each others roles to be able to provide cover. This has resulted in the quantity and quality of the data held by the team improving significantly. 

Bexley Learning Disability Team, Adult Learning Disability
A service user nominated the whole Bexley Learning Disability Team and Mencap for giving him hope for the long run.

Bill was nominated by a colleague for going the extra mile during the snow at the beginning of the year and for always giving 110%.

Bonnie Chandler and Kristy Chavura, Bexley CAMHS
Bonnie was nominated by a young service user for being "very kind and helpful" and because "they get me and mummy to play with each other more."

Bosky Nair, Greenwood Ward, Forensic and Prison Services
A colleague nominated Bosky for being a key contributor to the team, balancing study and excellent care.

Bracton Urban Art Project, Bracton Centre
An innovative project involving service users creating urban art, which is now on display in the Bracton Sports Hall. 

Bridgeways Day Hospital, Older People Mental Health
The team manager nominated the Bridgeways Day Hospital for the great strides it is making to improve the quality of care provision to older people with mental health problems. The nominator referred to an exciting new group programme the team has implemented which aims to build confidence, develop self worth and a purpose in life. 

Bridgeways Intake Team, Bromley Older People Mental Health Services 
Since the team was launched in April 2010 they have developed a system to deal with all referrals to mental health services for older people in Bromley borough. The nominator said "This team has enabled us to provide an excellent service to service users and carers. GPs have commented on how helpful it is to have a single point of access and named doctors who can provide support if needed."

Bromley Rehabilitation Support Team, Complex Needs and Recovery
A colleague from another team nominated the team because of the high standard of care they provide and their work in: supporting service users to experience new social activities, like the theatre and bowling; the speed in which they can respond to service users; and the support they provide to service users moving to less supported housing and own tenancy flats. 

Carer Education Group, Older Adults Community Mental Health Team - Greenwich
Sue, Joe, Afrose and Gill were nominated by a carer who has attended the Carer Education Group for service users with dementia and their families. The nominator took a lot out of the course and wanted to recognise them for being supportive, caring and dedicated. 

Carole Daniels, Greenwich Community Learning Disability Team
Carole is an occupational therapist and was nominated for her innovative work to increase the opportunities for employment and vocational training for people with a learning disability. She has increased the knowledge of colleagues. The nominator said: "The level of dedication, professionalism and initiative has been breath taking. She has made a huge difference to the service and an amazing positive difference to service users."

Caroline Pickup, Bexley Complex Needs and Rehabilitation Service
Caroline was nominated by two colleagues for her contribution to the standard and variety of interventions available to service users, improving their overall experiences. She has developed a gardening project at Goldie Leigh which enables service users to produce healthy food and build connections with the wider community. 

Challenging Behavioural Unit Occupational Therapy Team, Memorial Hospital
The team were nominated for: catering and hosting the Tony's Walk event in March; starting a monthly food stall in the hospital's reception selling homemade soups, snacks and cakes made by the service users and grown in their own garden; and producing a magazine featuring service user's creative writing, art and stories, as well as therapy programmes and cookery recipes.

Child Health Department, Bexley Community Health Services
The team were nominated for their work to improve information about children's health which has improved services and outcomes for families. The nominators said "The team are extremely flexible according to the needs of the service and have over the past year ensured that they are all able to cover a variety of roles which ensures that there is no backlog of information available to the  healthcare team due to annual leave or sickness."

Christiana Olomo, The Bevan Unit
Employed as Activities Coordinator in The Bevan Unit, Christiana has helped improved the overall satisfaction of patients since joining the team. 

Communications Team
The Oxleas Communications team were nominated for making "a significant difference to the extent to which ALD service users are included in trust initiatives and developments." This includes developing accessible staff recognition awards forms, membership forms and easy to read leaflets. 

COPD team, Greenwich Community Health Services
The service manager nominated the team for reducing unnecessary hospital admissions and increasing the service hours/days due to patient need. The team were also nominated for developing a GCHS wide coordinated service for the delivery of domiciliary oxygen for both adults and children. 

Crisis Resolution Home Treatment Team
The team were nominated for consistently providing an alternative to hospital admission for service users in Bromley throughout a time of change.

Curtis Michael, Bracton Centre
Curtis is a Support Time and Recovery Worker for Birchwood Clinic. The person who nominated him said: "Curtis has proactively supported service users to access education, employment and leisure facilities in the community. He has effectively been a "bridge builder" for our service users in facilitating their transition into the community in a safe and responsible way."

Cynthia Fong, Greenwich Forum District Nursing Team
Cynthia was nominated for her lifetime of service (she joined the NHS in 1970) and for 30 years spent in community nursing in Greenwich. She practices excellence in her own work and leads by example, sharing her knowledge and displaying great mentoring skills.

Danielle was nominated by a service user for looking after him, helping him to make his bed and to shave. 

Danson Clinic staff
The Danson Clinic staff were nominated by a number of Bracton Centre managers for their excellent work in a number of areas including: Healthy Lifestyles; Positive Slant; Danson Magazine and the Productive Mental Health Ward. They were also nominated for having a user focus and involving residents and their families in all aspects of their care.

Nothing is too much trouble for volunteer driver David. The colleague that nominated him also said he showed a lot of care and understanding of the patients.

A colleague nominated Debbie for putting service users at their ease and for working very hard to do everything possible for them.

Debbie Green, Complex Needs Recovery Service - Greenwich
Debbie was nominated by a colleague for the work she has done around social inclusion and promoting the concept of recovery in every area of clinical and policy led work. The nominator said: "She advocates and explains what recovery is in the true sense of the word and works to challenge and influence others in the pursuit of recovery orientated practice."

Debbie was nominated by her manager who said "without her we could not provide such a high quality service…She is unfailingly pleasant to complainants who quite often are angry or distressed, to staff when chasing up information that has not always been sent on time - and to me when I make the umpteenth request for yet another amendment to a letter or ask for another report."

Debra King, COPD Team, Greenwich Community Health Services
Debra plays an important role as the first point of contact for people with chronic obstructive pulmonary disease (COPD), arranging their supplies of oxygen. She was nominated by the COPD Team Leader who said "she has consistently performed above the level expected of her." She has improved the team's efficiency and accurate data collection and often goes beyond her duties for the benefit of a patient. For example she stayed at work late to contact utility companies to ensure a patient returning from hospital had heating and light.

Diane was nominated for dedicating 25 years to the NHS.

Dolly Bartlett, Day Treatment Team at Oxleas House
Dolly was nominated numerous times by colleagues for her hard work and excellent attitude. One colleague said: "She is an inspiration to me as a nurse for her positivity, adaptability and is an excellent role model in nursing."

Don Boyle, Employment Team - Bexley
A governor nominated Don as he has been impressed and inspired with his expertise and deep, devoted interest in meditation to help service users in their personal rehabilitation.

A service user nominated Dr Moriarty for showing her how to keep her anger down.

Dr Anthony Akenzua, Oxleas House
Dr Akenzua is the Clinical Director in the Adult Acute Mental health directorate and also a Consultant Psychiatrist at Oxleas House, our inpatient unit in Greenwich. Since becoming clinical director he continues to set an example to staff from all professions at Oxleas House. Anthony was nominated for his leadership on a ward with the highest number of admissions and discharges. He was described as "a team player, always willing and encourages views and opinions from all staff, patients and family".

Dr Claire Green, Primary Care Dental Service
A colleague nominated Claire for striving to improve the service through additional training in sedation dentistry and for being a team player.

Dr Colm Long, Short Term Interventions Team - Bexley
A mother and carer nominated Dr Long for helping her and her son manage his illness. The nominator said that Dr Long is the only person her son will cooperate with. He has also helped her to understand her son's illness.

A colleague nominated Dr Tracy for being down to earth and treating people with compassion.

A colleague nominated Dr Martins for her thorough assessment of clients making sure social factors are explored early in admission which allows others to do their jobs well.

Dr Inglis was nominated by colleagues who said "He is very approachable and easy to get along with. He works very hard and always takes the time to ask colleagues how they are…He is one of the best. Excellent."

Dr Nagendran was nominated by a service user for "being so open to me."

Dr Omer Malik, Betts Ward Greenwich Parks House
A colleague from another service nominated Dr Malik for being reliable, hard working, compassionate and trustworthy. The nominator noted that Dr Malik puts his responsibilities first and will make decisions based on the needs of the ward.

Sergei was nominated by a service user for listening and giving good advice and for "being the best doctor anyone could ever wish for."

Eleanor Johnston, Bridgeways Day Hospital
Eleanor has worked in the older adults service for the past five years and her current role is Community Manager. She was nominated by several colleagues who said "During this time she has met the challenges and changes within the service and has always strived to  have the needs and views of the service users as her main focus." She has reduced waiting times and supported and encouraged colleagues to develop their skills.

Elizabeth was nominated by a patient for being helpful, punctual and always having a smile on her face. 

Eltham Forum, Integrated Community Team, Greenwich Community Health Services
The service manager nominated the Eltham Forum for the positive changes the team has made to enhance care for patients since integrating district nursing and podiatry services. Some of the changes highlighted were having a named nurse for every GP surgery, implementing monthly forum meetings and engaging in productive community services. 

Emma Moody, Human Resources
Emma was nominated for her hard work and dedication to get the Allocate E-Rostering system off the ground in Greenwich Community Health Services. She was commended by the nominator for how well she worked with different teams and the software consultants to ensure the system would meet the needs of different teams. 

Ethel Nazzal and Frederica Joseph, Greenwich Recovery East Team
Ethel and Frederica were nominated by colleagues for leading a healthy body, healthy mind project which has made a positive difference to service users' physical health and improved relationships with carers and service users.

Eva Douglas, Shepherdleas Ward, Oxleas House
Eva was nominated by a colleague who said "she looks after her patients very well and with a loving heart." She is praised for supporting colleagues and ensuring that the ward remains safe. The nominator again: "I for one have gained a lot working with Eva and I have a lot of respect for her professionalism." 

Family inclusive practice care trainers
The carers who talk about their experiences of being a carer of someone with mental ill health were nominated by a member of staff for their bravery, wit and generosity in sharing their personal experiences with staff as part of this training.

A member of Fiona's team nominated her for her ability to balance a demanding workload whilst still being a responsive, supportive and approachable manager. 

Gail Francis, District Nursing in Bexley Community Health Services
For being an excellent nurse and mentor to other nurses and an advocate to patients and nurses alike. 

Gallions Reach Health Visitors, Greenwich Community Health Services
The team were nominated for working hard to welcome and integrate new team members and developing work with partner organisations. The nominator said "The health visitors are also building links with the local GP surgeries, attending meetings and sharing information about families of concern to improve the care given to families by both GPs and the health visitor service."

Garland Road and Gallions Reach School Nursing Teams, Greenwich Community Health Services
The teams have had staff shortages and has been reorganised. According to the nominator: "During this difficult time the conscientious and hard working team have continued to provide an excellent service families, developing positive relationships with schools and responding to parents' concerns regarding the controversial regarding the controversial National Child Measurement Programme."

Garland Road Health Visiting Team, Greenwich Community Health Services
The nominator wrote that "The Garland Road Health Visiting Team work hard to provide an excellent service to local families spread over a wide area. They have experienced staff shortages over the past year but have continued to work well with local families and to develop partnership working with two children's centres."

Gertrude and John were nominated for successfully responding to carers' needs and for carrying out carers' assessments providing information and support.

Gill was nominated by a colleague for her ability to build relationships with clients, always having a smile on her face and for always helping staff and clients when they need it.

Gillian was nominated for being a pillar of support, helping the nominator make the transition from support worker to qualified nurse.

The entire team were nominated by a volunteer who was impressed by their professional excellence, their hard work and competence, and their warmth and kindness.

Greenwich Community Occupational Therapy Team,  Mental Health Services
The team were nominated for working with service users and film-makers, Inspirational Filmworks to produce a film based on the Foresight Project's five areas of wellbeing. The film contains information about gardening, occupational therapy and an original song composed by performed by members of the music group.

Coming up to retirement, a colleague wanted to recognise Hakim for his hard work in the 20+ years he has worked for the trust.

Hannah Whitfield, Hazelwood House Memorial Hospital
Hannah received two nominations. One said: "Hannah is a fantastic role model to service users and staff. She actively promotes social inclusion and is dedicated and committed to every aspect of her work."

Healthcare assistants in Bexley Community Health Services' District Nursing service
Colleagues nominated the band 3 healthcare assistants in the district nursing service for all completing training to enable them to give injections to named patients in their own homes. The nominator said they: "all embraced the training and help set a new precedent for other HCAs."

Healthcare assistants, School Nursing Team, Bexley Community Health Services 
The team were nominated for "working relentlessly in a joint effort to measure and record the height and weight of all children in Reception Year and Year 6 as part of the National Child Measurement Programme."

Heath Clinic, Bracton Centre
Heath is a low secure clinic at the Bracton Centre. Recently they have undergone significant changes as they have changed to become a female only ward and admitted more complex and challenging patients. The Health Clinic team received more than one nomination. One praised staff for their high levels of clinical practice and the way they have managed risk and reduced the number of incidents on the ward.  

Hilary Jakeman, Community Mental Health team
Hilary was nominated by her team for going above and beyond her caring role, particularly with regard to her support of one particular patient. She also walked for miles in the snow to visit vulnerable patients when staff were unable to safely use their cars during bad weather.

Hilary Radcliffe and Sue Lambert-Gregory, Greenwich Contraception and Sexual Health
A service user nominated Hilary and Sue for making her feel comfortable and cared for during "what is not a pleasurable experience as a rule."

HMP Rochester Day Centre Team
The team has worked extremely hard over the past 12 months during which they have developed an innovative stepped care service for young adult male offenders. This includes group programmes, self help materials and work to improve access to psychological therapies.

Horizon House team
A number of life members nominated Horizon House for the support and advice they give. One member said: "Horizon House is the most positive thing I have done to help get me back on my feet."

ICT Department, Bracken House
The Information Communication Technology (ICT) Team were nominated by the Director of Nursing and Governance, Wilf Bardsley who said "People get frustrated when their computer doesn't work or when they can't access their emails, but most don't realise the amount of sheer hard work it takes to keep the trust IT systems working 24 hours a day." He praised them in particular for their work when Greenwich Community Health Services joined Oxleas this year.

Jane Sweeny, School Nursing, Bexley Community Health Services 
Jane was nominated for being "innovative and forward thinking, and committed to providing quality services to young people." Not all young people find a face to face approach easy so Jane has implemented a text messaging service which provides open, confidential health related advice to students in Bexley schools. 

Janice Algar, Bexley Learning Disability Team
Janice was nominated by a service user for being very supportive "above and beyond the call of duty."

Jeanne Pretorius, Bexley Crisis Team
A colleague nominated Jeanne for demonstrating significant personal courage in managing a successful return to work following a number of surgeries that have affected her mobility. She has shown leadership qualities that have inspired her team. 

Jennifer Cotton, District Nursing (bank) Bexley Community Health Services
Jenny retired a couple of years ago after setting up some district nursing clinics for patients who could leave their homes but required specialist care their practice nurse couldn't provide. She has since returned to work part-time on the bank in order to run a "scaled down" version of the clinics. Her colleague that nominated her said that her clients described her in the "most positive and professional way" in a recent audit.

A colleague nominated Joanne for being an excellent role model. She encourages the team to take on new challenges and keep abreast with practice developments.

John was nominated by one of his staff who said "John has been the most proactive and supportive supervisor. He has continued to support me in handling my workload and also given space and guidance for my personal professional development."

Johnny Measures, Danson Clinic, Bracton Centre
Johnny was nominated by a service user for his communication, explaining things clearly to him and keeping him and his family informed about his care. 

Joint Emergency Team, Greenwich Community Health Services
Launched in April 2011 as an amalgamation of the GCHS Rapid Response Team and London Borough of Greenwich Adult Social Services Emergency Intervention Team, the team has forged great working relationships with local GPs and A&E staff. The nominator said they: "always put patients needs first in incredibly challenging circumstances."

Jonathan was nominated by a service user she said that "I always  felt assurance and confidence when he was around." She also praised him for being selfless, caring, responsive and a good listener.

A GP nominated Jonathan and the rest of the team for the support they gave to her son, transforming the lives of their family.

Josephine was nominated by a colleague for the role she plays in promoting the health and wellbeing of patients on the ward.

Joyce Sekyere, Older Adults Community Mental Health Team - Bromley West
Joyce was nominated by a service user for all the help and support she gives. The nominator said: "Nothing is too much trouble for her."

The patients at Joydens Clinic nominated the staff for having a user focus, making them feel welcome and for running a number of activities and sessions.

A colleague nominated Julia for a clinical approach she designed and developed, which has now been earmarked to be rolled out in other social care teams.

A member of Julie's team nominated her for leading change within the service, whilst still leading and supporting the team.

Kathy, Julie and Diane were nominated by colleagues for their efficiency, warmth and for being welcoming with an "incredible ability to put the families attending the service at ease."

Julie was nominated by a couple of colleagues. One nominated her for not only doing her day job, but organising the new reception and training staff on it.

Joyden's Clinic Ward Manager nominated Julie and Matthew for their outstanding work with the Dear Diary Group. The nominator said "people like them are the unsung heroes of Oxleas".

Julie Spink, Rehabilitation Support Team Bromley
Julie has set up and overseen the development of a physical health monitoring clinic for clients in the service who would otherwise find it difficult to attend mainstream services for healthcare monitoring. In doing so, she has been able to identify physical healthcare problems in some patients that would have otherwise gone undetected.

For ensuring that the cleaners do their jobs well, all patient rooms are clean and have fresh bedding. The nominator also said she encourages him and the other service users to look after themselves. 

Kelly Kooner, Clozapine clinic, and Mohamed Mohoboob, Recovery East Team - Greenwich
A service user nominated Kelly for being approachable and professional and Mohamed for "always being there." 

Kelly West, Greenwich Recovery Team
Kelly was nominated by a consultant psychiatrist and her team manager who said she "routinely performs at the highest level. She is very popular with our clients and staff. She has clinical skills of a high standard. She is creative and resourceful."

Kerri Newns, Greenwich Child and Adolescent Mental Health Services (CAMHS)
Kerri was nominated for the innovative work she has done with Greenwich and Lewisham Young People's Theatre. They ran an arts programme for young people referred by CAMHS who developed their acting, listening and relationship skills. The young people put on a play. Benefits included improving access to CAMHS, de-stigmatising mental health issues, promoting social inclusion and meeting the needs of Black and Minority Ethnic groups.

Lauren Harman, Recovery Team - Greenwich
Lauren was nominated by a service user for helping her get back out into the world through volunteering and for helping her find a new church to go to. 

Lawrence Yong, Woodlands Unit
Lawrence was nominated by a colleague who has known him for over 15 years for his hard work, understanding, empathy and dedication.

Learning Disability Admin Team, Bexley
The team were nominated for stepping up to the plate and taking on additional roles, responsibilities and projects with the separation of health and social care within the service. 

Lesley Doolan, Older Adults Community Mental Health Team - Greenwich
Lesley received a couple of nominations. She is often the first point of contact for people ringing into the team and has developed a great rapport with service users. The "Angel" as she is widely known, has a great knack for cheering service users up and they "often forget why they were calling." 

Linda Peck, Complaints Team, Pinewood House
Part of Linda's job is to answer the job to people who want to complain about the trust. She was nominated by her manager who said "She deals all day with people who are unhappy, angry and distressed because they feel they have not been treated well by the trust. Linda is unfailingly friendly and sympathetic and often manages to give patients and/or carers the immediate help they need...Unusually for someone working in a complaints department, Linda actually receives letters of thanks from complainants."

Lynn was nominated by her manager who said she is an "outstanding leader" and listed several successes that the chronic obstructive pulmonary disease (COPD) team have achieved under her leadership. These include being listed in Department of Health guidance as an example of good practice; providing an out of hours service; managing the oxygen service and decreasing spend; increasing the flexibility of the service; working better in partnership with GPs; and piloting telehealth.

Maggie was nominated by a colleague for the role she plays in helping clients to remain in their place of work, by negotiating with and on behalf of clients who have difficulties dealing with these issues whilst unwell. Maureen is the lead infection control nurse on Norman Ward, which is the best ward for infection control in Greenwich Parks House because of her. The colleague who nominated Maureen also said she always shows empathy, dignity and respect.

Melanie was nominated for "transforming the health visiting part of the service in the 20 months since she has been in post." When she came into post sickness rates and morale were low and recruitment of staff was a challenge. She has developed existing staff skills and reduced vacancies to none and the service has improved dramatically.

Michelle took on the role of Immunisations Team Leader on top of her school nursing caseload and has led the team to reach its CQUIN target for 2010. Michelle has also taken the time to re-evaluate the team's policies, encouraged staff into further training and established better links with the community.

One nominator said: "he has a special way with clients, and defuses any situations that may arise. Oliver was nominated by a colleague for "constantly striving to provide client centred occupational therapy within an extremely challenging setting." He has made considerable improvements to the service on the ward he works and also to the directorate as a whole.

Olivia was nominated by a carer for her work in transforming the way the carers' son manages his emotions. The "huge contribution" she has made to the family's life has led to a much more harmonious and manageable atmosphere in their home.

Debbie, Maggie and Sarah (the Oxygen team) were nominated by their service manager for the work they have done to develop a coordinated service for the delivery of home oxygen, supporting GPs and other colleagues around completing Home Oxygen Ordering Forms (HOOFs) and being a reliable first point of contact for patients.

Patricia was nominated for taking on the lead role of managing the continence service in Bexley in addition to her role as a district nurse at the Oval Clinic. She is passionate about her work and trains and supports colleagues. The nominator said "she offers expert advice to patients, carers and other healthcare professionals."

Paula was nominated by some of her colleagues for developing a pharmacy clinic which has enabled the team to give greater scrutiny to medication and enabled clients to have more meaningful discussions about their medication.

Peter was nominated by a colleague for his diligent work in creating a strong, dedicated and uniform team. He is praised for being a excellent example of a team leader, able to work with people and succeed in ensuring the team delivers high quality care. He was also nominated by a service user for being pivotal in his recovery. The service user said: "He has shown me how to trust, and love what was once myself again."  

Phil Friel and Tony Tang, Rehabilitation Support Team, Blean Grove
Phil and Tony were nominated by a service user for their work over the last six years setting up and running the Bromley Music Group in which people are able to play and create music together. The group has provided a friendly and supportive environment in which people have fun and build their confidence and sense of wellbeing, facilitating their recovery and integration back into their local communities.

Pulmonary Rehabilitation Team, Greenwich Community Health Services
The team were nominated for enabling patients to make informed choices about their health. They run courses for patients which enable them to do more physical activity and have a better understanding of their health needs. Feedback from patients is very positive.

Ramanand Shamy and Vicky Tosun, TOPS Day Service
Ramanand and Vicky work in the older person's day service (TOPS) for people with a learning disability. They were nominated by a service user for the way in which they help them onto transport with care.

Respiratory Nurse Specialist and Virtual Ward, Bexley Community Health Services
A colleague in Bexley Community Health Services nominated the respiratory nurse specialist and virtual ward for their work to implement telehealth, a vital stats monitoring system that sends information over the telephone line from the service user's home to a clinical team, in Bexley Community Health Services. The feedback from service users using telehealth has been very positive. The nominator commented that changing practice can be challenging and that she was extremely proud of the way the team have led by example.

A service user nominated Rosanne for being compassionate and dedicated, coming in with shopping and dry cleaning outside of her working hours for service users. 

Roy Norris, Bromley Recovery Team West
Roy was nominated by a colleague who said "he is always one of the first people in work each day and often the last to leave." He is always eager to help his colleagues and he works unpaid during the weekends to cook meals for patients.

Sandra is a senior Approved Mental Health Practitioner and Deputy Team Manager for Recovery Team East. The nominator said that she enabled and encouraged her colleagues to take advantage of learning opportunities.

Sarah was nominated by a service user for helping her get back out into the community and for being a good listener and communicator. 

Sarah Hadley and Alexia Quinn, Music Therapy Service
Sarah and Alexia were nominated for developing an interactive music making learning module for people working in early years settings, which is now run by the University of Greenwich.  

Sarah Paynter, Complex Needs Recovery Service - Greenwich
Sarah was nominated by a service user for helping her to achieve a sense of achievement, take on a voluntary role in Oxleas and manage her anxiety. The nominator said: "without her ongoing support I would not have been able to manage going out on my own."

A colleague nominated Sarha for her ability to reach out to, communicate with and support hard to reach and vulnerable clients in the Colyer's Wood area, which has a lot of deprivation. 

School Nursing, Bexley Community Health Services
The School Nursing Team were nominated for work to improve health of children in schools with a medical condition. The nominator said: "They developed a working group which included parents, teachers, Bexley Council, public health, the healthy schools coordinator and a paediatrician from South London Healthcare Trust." They created a medical alert booklet template for schools to produce to enable them to share key information and improve access to emergency care when required.

A service user nominated 'Shell' for being "a good listener and agony aunt who concentrates all her energy to whatever task."

Shrewsbury Lane Team, Greenwich Complex Needs Recovery Service
Shrewsbury Lane provides supported accommodation recovering for mental ill-health. Staff were nominated by a doctor for being "dynamic, creative, innovative and taking on new challenges." The team attended British Sign Language training and have worked to improve their understanding of autistic spectrum disorder.

Shrewsbury Ward Team
The team on Shrewsbury Ward received 12 nominations for the support they provide to service users on the ward. They were described as: "caring staff who acknowledge you", were praised  for their professionalism, help and understanding, and for providing excellent care in an atmosphere of camaraderie. Tribute was also paid to the vital coping skills they teach people and for the atmosphere of safety, security and equality that they have built on the ward.

Sickle Cell and Thalassaemia Service, Public Health Community Nursing, Greenwich Community Health Services
The team were nominated by a colleague who said they "have worked tirelessly to provide a comprehensive, high quality service to adults and children who have been diagnosed with a haemoglobinopathy disorder, or whose baby may be diagnosed with a disorder. Their services range from clinic consultations to home contacts for families in both Greenwich and Bexley boroughs...The team are conscientious, committed and believe wholeheartedly in the benefits of their work."

A service user nominated Mr Frank Webb and his team for providing the whole package (assessment, treatment and aftercare) with skill and a smile. 

Stacy Washington, Lead Nurse for Practice Improvement, Pinewood House
Stacy was nominated for setting up and coordinating two annual staff wellbeing events. The events have included stalls on exercise and physical activity and a range of health and wellbeing checks for staff. They have been well attended by staff with positive feedback and requests for repeat events.

TaMHS School Nursing Working Party
For helping young people address their mental health problems enabling them to achieve their educational, emotional and social potential. 

Tara Dinsmore, Oxleas House
Tara is an occupational therapist at Oxleas House, our mental health inpatient unit in Greenwich. She was nominated by a doctor who said she is "an energetic and enthusiastic person who plays a very important role in motivating and enabling patients to engage with activities on the ward, to reconnect with life outside and to plan for the future." Tara runs a relaxation group on Tuesday evenings on the ward, increasing the activities available in the evening and helping people to manage anxiety. 

Teresa McDonald, School Nurse Team, Erith Health Centre
Teresa is the team leader for the School Nurse Team based at the Erith Health Centre. She was nominated by staff from the team who praised her "excellent leadership" for being "innovative and forward thinking" and for her "good communication skills".

The Bevan Unit Falls Steering Group, The Bevan Unit
The Falls Steering Group at the Bevan (our intermediate care unit in Thamesmead) have worked very hard over the past 18 months reducing the number of falls at the unit from an average of seven per month to 2.25 per month. The multi-professional group have improved falls assessment, started using a seven step checklist and improved medication reviews.

A colleague nominated Tola for providing "the highest standard of quality care." She has consistently gone beyond the call of duty,  supporting families and ensuring that service users are involved in decisions affecting their care.

Twilight District Nursing Service, Bexley Community Health Services
The team have led on the community productive initiative in Bexley CHS and transformed their working environment enabling them to work more efficiently. The team work between 5pm and 11pm every day visiting patients at home. The nominator said that "The team demonstrated practice over and above what is expected of them during the snow this year, walking to patients' houses to ensure they received appropriate care night after night."

A service user nominated the team for all of the care and consideration they give, Samantha for all the help she gives and Michael for driving her to the hospital and for always being cheerful.

A volunteer in the Upton Day Centre nominated the rest of the team for making her feel like she was part of the team and  helping to give her more confidence. The nominator said that the team has: "turned my life around." They were also nominated by a service user who said that a day there is "so stimulating and fun."
