Community Assessment and Rehabilitation Greenwich

The Community Assessment and Rehabilitation teams work in partnership with Greenwich Council to provide community-based assessment, rehabilitation and prevention services for people over 18 years of age.

Services provided include:

  • Neuro-rehabilitation, for injuries and conditions including head injuries, Stroke, Parkinsons, Cerebral Palsy and MS and Motor Neurone Disease
  • Dietetics, for people who require dietary and nutritional advice and support
  • Mobility assessments and intervention
  • Assessment and rehabilitation of activity of daily living (ADL), with a reablement focus
  • Falls intervention
  • Help to manage a long-term condition.

How to access this service

Referrals are taken directly from patients who need the service and from health and social care professionals.
