The Joint Emergency team assesses people, aged 18 years and over, in their own home, A&E and the Acute Medical Unit (AMU) at the Queen Elizabeth Hospital, to:
plan and arrange rehabilitation and ensure it is undertaken in the appropriate setting ie in their own home or in an intermediate care facility, with the patient’s best interests at heart.
The team work closely with social services, Queen Elizabeth Hospital and the three intermediate care units, the Bevan Unit, Artillery Unit at Ash Green and the Ross Unit at Time Court.
The team will see people that require assessment within 24 hours.
The Joint Emergency team accepts referrals from all primary, community, acute and social services. This service is available to Greenwich residents or those registered with a Greenwich GP. Referrals are made through the Joint Emergency Team
